Enrolling in Keystone STARS
To participate in Keystone STARS, you must complete an Enrollment Application (click here) and provide the following to the NW Regional Key:

Copy of your current DPW Certificate of Compliance or Registration (visit the Department of Public Welfare’s website for more information if needed at http://www.dpw.state.pa.us ).
A Keystone STARS Representative will be assigned to your program upon receipt of a completed application and will contact you to get started. Please contact us with any questions or concerns.
